Should I Modify the Subject Header or Add a Header with Anti-SPAM?

It is recommended adding a header and filter based on that header into a folder on the mail client. The reason for that is if you were to check the SPAM folder and there was a false positive that you needed to reply to, the recipient would not notice anything different with the message. If you were to filter based on Modifying the Subject Header, the text added to the Subject Header would be included and you would have to manually delete the text when replying to the message.
